To start creating a Form, select Global settings and click on Forms. When creating questions in a form, you can choose between the following types of questions:
- Yes/No
- Free text
- Multiple choice (checkbox)
- Single choice (radio button)
- Single choice (dropdown menu)
Select the question type in the dropdown menu with the corresponding name and decide whether the question should be mandatory by checking Mandatory. To the right, you can preview how the question will look to the patient.
Yes/No
The Yes/No question type is binary and cannot contain more than two options.
Free text
With the free text type, the patient can answer the question in continuous text. The text field can contain several lines of text. The text you write in the Placeholder field can be used to clarify what the patient is expected to write and is overwritten when the patient responds.
Multiple choice
Multiple choice means that the patient can select one or more options.
You can choose to have only one option if you for example want the patient to confirm that they have read certain information.
Single choice (radio button or dropdown menu)
The single-choice question type has two versions, radio buttons and dropdown menu. Both versions work in a similar way where the patient can only select one of the options.
