As a global administrator, you can create and modify forms that can be used by all clinics within your business. You can also link forms to a specific clinic, modify forms and delete forms.
Forms can be used to get information from a patient before a video consultation or for evaluation after the visit.
To create a new form, follow these steps.
- Go to the Forms category from the sidebar menu to the left. If a form has already been created, it appears on the list.
- To create a new form, click on the plus sign from the lower right corner.
Add a name and a description of the form. Please note that, if you have activated multiple languages, a name and description needs to be added for all of them. Click on Create first page.
- To manage the questions included on the first page of the form, click on Create first question. Choose the type of question (Yes/No, Free text, Multiple choices, Single choice, Single choice drop-down list) and formulate the question.
- If the question is a free text field, fill in the Placeholder field under the question. The text you add in this field will be displayed in the answer field and is overwritten when the patient responds. To the right of the interface, you can see how the question will look to the patient.
- To add more questions to the form, check the Add another question box to the bottom left. Click on Save.
- Formulate the next question according to the instructions above.
- When you have added the last question, make sure that the Add another question box is unchecked. Click on Save.
- To add a new page, click on the plus sign at the right corner of the interface.
- To make the form available to other clinics, drag the slider under Availability to Publish. It will then be placed in a "forms library", where the clinics can choose to activate the forms they want to use.
- To remove a question, hover over the far right of the question and click on the x symbol.
